What is MESA?
Micro Epididymal Sperm Aspiration (MESA) is a precise surgical technique used to retrieve sperm directly from the epididymis — a coiled tube where sperm mature and are stored. This method is particularly useful in men with obstructive azoospermia, where sperm production is normal but a blockage prevents sperm from appearing in the ejaculate. The sperm collected through MESA is typically used for IVF (In Vitro Fertilization) or ICSI (Intracytoplasmic Sperm Injection).
When is MESA Recommended?
MESA is often advised in the following cases:
- Failed Vasectomy Reversal: When natural sperm return is not achieved.
- Obstructive Azoospermia: Due to trauma, infections, inflammation, or previous surgeries.
- Congenital Absence or Blockage of the Vas Deferens: Often related to conditions like Cystic Fibrosis.
- High Sperm Quality Needed: MESA provides a large number of mature sperm, useful for future IVF/ICSI cycles.
Steps Involved in MESA
- Anesthesia: MESA is performed under general or local anesthesia, ensuring patient comfort.
- Microsurgical Access: A small incision is made in the scrotum to expose the epididymis.
- Sperm Aspiration: Using a high-powered microscope, sperm are carefully aspirated from the epididymal tubules.
- Laboratory Processing: The extracted sperm are processed, evaluated for quality, and either used immediately or cryopreserved for future use.
